New report shows high, sustained vaccine effectiveness against severe COVID-19

A new weekly morbidity and mortality report shows a small change in incidence rates for hospitalizations and deaths from severe COVID-19 after the SARS-CoV-2 delta variant gained dominance in 13 US jurisdictions.

From April 4 to July 17, there were 569,142 COVID-19 cases (92%), 34,972 COVID-19 hospital admissions (92%), and 6,132 COVID-19-associated deaths (91%) among adults who were not fully vaccinated. In fully vaccinated adults, there were 46,312 COVID-19 cases (8%), 2,976 COVID-19 hospital admissions (8%), and 616 COVID-19-associated deaths (9%).

When the researchers analyzed the earlier April 4 to June 19 period, 5% of COVID-19 cases, 7% of COVID-19 hospital admissions, and 8% of COVID-19-associated deaths were reported in fully vaccinated adults. These percentages increased to 18%, 14% and 16%, respectively, from June 20 to July 17.

From April 4 to July 17, the weekly prevalence of the SARS-CoV-2 delta variant rose from less than 1% to 90%.

Full COVID-19 vaccination coverage also increased from 19% to 54% during this period. From July 10-17, the range of full immunization coverage ranged from 45% in adults aged 18 to 49 years to 73% in adults aged 65 years and older.

“After the week of June 20, 2021, when the SARS-CoV-2 Delta variant became predominant, the proportion of fully vaccinated people in the cases increased more than expected given the given vaccination coverage and constant vaccination effectiveness,” said Heather M. Scobie, PhD, of the Epidemiology Task Force for the CDC COVID-19 Response Team, and colleagues wrote on the MMWR Weekly Report.

The mean weekly age-standardized incidence rates (IRRs) from April 4 to June 19 were higher in not fully vaccinated adults than fully vaccinated adults, including reported cases (112.3 vs. 10.1 per 100,000 people), hospital admissions (9, 1 vs. 0.7 per.). 100,000 people) and deaths (1.6 vs. 0.1 per 100,000 people) and from June 20 to July 17 (89.1 vs. 19.4 per 100,000 people, 7 vs. 0.7 per 100,000 people or 1.1 vs. 0.1 per 100,000 people). The age-standardized IRR for cases of not fully vaccinated persons compared to fully vaccinated persons decreased from April 4 to June 19 compared to June 20 to July 17 from 11.1 to 4.6; The IRR for hospital stays (down from 13.3 to 10.4) and deaths (down from 16.6 to 11.3) also decreased between these two periods.

“The results were consistent with a potential decline in vaccination protection against confirmed SARS-CoV-2 infection and continued strong protection against COVID-19-associated hospitalizations and deaths,” wrote Scobie and colleagues.

The 13 U.S. jurisdictions were Alabama, Arizona, Colorado, Indiana, Los Angeles County (California), Louisiana, Maryland, Minnesota, New Mexico, New York City (New York), North Carolina, Seattle / King County (Washington), and Utah.
